Project Synopsis

The clinical laboratory plays a vital role across the entire continuum of health care. Up to 85 percent of diagnosis and treatment decisions are based on laboratory results. Hospital and physician practice models are moving to fully integrated services. Inpatient, outpatient and outreach test results will need to be fully integrated within these new practice models. An integrated health system laboratory will play an increasingly important role in decreasing unnecessary testing and its associated downstream medical costs. Their hospital’s current set-up does not support a fully integrated diagnostic service. Besides, due to lack of space they are unable to bring in advanced equipment that could meet the quality delivered through fully integrated laboratory service and in fetching accurate results, and reducing the turnaround time. Hence Hindu Mission Hospital plans to set up a fully integrated laboratory and blood transfusion centre

Proposed Project Description

Project Introduction:

The pillars of an excellent hospital includes clinical services, laboratory services, radiology, and blood bank. Technology must keep pace with time else it risks becoming obsolete. The benefit is that the average or poor person is no longer constrained due to lack of availability or affordability.

 

Good laboratories do much more than just run a specimen through an analyzer to get a result. They determine which tests should be offered to support the medical staff, how they should be built electronically so that the proper test is ordered and how they are reported so that results are interpreted correctly. Laboratories make certain that appropriate tests are included in clinical order sets and unnecessary tests are deleted. Hospital laboratories perform testing as quickly as possible so that diagnosis and therapeutic decisions are not delayed. They carefully monitor laboratory quality to ensure accurate and reliable test results.

 

The humble laboratory, once confined to the bowels of the hospital and operating in confined silos has evolved into an equal player at par with other services. The aim is to make an accurate diagnosis in quick time using multiple components to plan patient management. This is aptly labelled an “Integrated Laboratory Service” where the whole is more than a sum of its components. The components of a standard laboratory service include hematology, biochemistry, microbiology, and pathology. Added to this in over the last decade are specialties such as transplant immunology and molecular biology. This has not only made it possible to accurately detect diseases but also helps select individualized treatment options depending on the patient’s genetic makeup. This provides increased efficacy while minimizing adverse effects of therapy.

 

As their hospital strives towards making complex hepatobiliary pancreatic surgery, gastrointestinal oncology, and organ transplantation available to a wider section of the society particularly the underprivileged at grossly subsidized rates, they need precise and accurate tools to help achieve results comparable to any hospital or program in the world.

 

Components of Standard Laboratory service:

Blood Transfusion Services (Hematology):

The traditional blood bank has transformed into a specialty, transfusion medicine. A modern blood bank does not issue whole blood anymore. On collection, blood is tested for infections and separated into its components; packed red cells, plasma, platelets, and cryoprecipitate and stored. Patient’s coagulation is measured using dynamic tools and the deficient component is transfused lowering the risks of transfusion related allergic reactions and complications.

 

Biochemistry:

Biochemistry focuses on processes happening at a molecular level. It focuses on what is happening inside our cells, studying components like proteins, lipids and organelles. It also looks at how cells communicate with each other, for example during growth or fighting illness. Biochemists need to understand how the structure of a molecule relates to its function, allowing them to predict how molecules will interact.

  

Microbiology:

Microbiology has 3 main arms: bacteriology, virology, and mycology. Viruses are being recognized as the causes of a host of diseases including viral hepatitis and the now infamous coronavirus. Early detection, timely treatment with effective antiviral medication can reduce the risk of progression to advanced multiple organ diseases and other diseases in the body. Early detection and quantification of viral load with timely treatment is a key step in this process. Identifying the organism and its antibiotic sensitivity helps choose the most suitable antibiotic, preventing overuse or misuse.

 

Pathology:

The pathology laboratory has also evolved from traditional processing of surgical specimens to a more active role. Collaboration with clinical fields have been reported to increase the chances of identifying and detecting pathology early for directing management.

 

A fully equipped modern pathology facilitates advanced procedures for diagnosis. For eg: The frozen section procedure is a pathological laboratory procedure to perform rapid microscopic analysis of a specimen. It is used most often in oncological surgery. The technique of frozen section evaluation is important for guiding extent of surgical removal of tissues or organs suspected of being involved by cancer. This technique can process a specimen in just 20 minutes compared to 1 day for conventional tissue preparation techniques. Cancer tumors are further analyzed for further mutations that make them responsive or unresponsive to specific chemotherapeutic or biologic agents. This helps patients save valuable time and offers the best chance of obtaining maximum response while minimizing side effects.
