About the organization
ABMSS is a pan India organisation, set up in the year 1993 in Bangalore to promote the social causes of child and people welfare. Over the years the organisation has morphed from a catalyst, supporter, promoter and sponsor for so many causes. Since 2013, the organization went on to add the cleft lip and palate treatment as one of its major programmes. It started with just two projects in Coorg and Mysore and has now grown into a pan India presence. Apart from the cleft lip and palate, the organisation also supports awareness work in rural areas; education of doctors and medical personnel involved in supporting the charitable cause; speech and my-functional therapy; supplementing with vitamin and minerals to pregnant people and malnourished children.
ABMSS is a non-profit organisation in India registered under Karnataka Societies Act, 1960 with Reg.No:495/93-94 and Government of India, NITI Ayog, with ID: KA/2017/0163612. The Society has valid 12A, 80G and FCRA registrations.
- ABMSS provides, comprehensive cleft care, which comprises of surgery, orthodontic treatment, ENT, speech therapy and functional therapy.
- These surgeries are costly when conducted in private hospitals. Single Intervention of Cleft Surgery costs more than Rs.1,00,000/- in normal conditions, without comprehensive care. With Cleft India Project, it is minimized to Rs.19,500/- per surgery with comprehensive care.
- 70% of cleft patients have both cleft lip and cleft palate [CLP]: A large number of CLP patients now depend on ABMSS for secondary surgery as hardly any other organizations offer comprehensive cleft care though there are a number of mission based organizations which just offer a single surgery and not the complete circle of cleft care.
- Cleft lip and palate is the most common congenital facial deformity in the in the world: It is less known fact that every year about 35,000 children are born with a cleft lip and/or palate.
- In India there are more than 10 lakhs untreated cleft patients: Children with the untreated cleft face severe health problems [drinking, eating, speaking and hearing], suffering from social exclusion and sadly one out of ten of these children hardly get to see their first birthday.
- There are many causes of cleft lip and/or palate. Problems with genes passed down from one or both parents, drugs, viruses, or other toxins can all cause these birth defects. Cleft lip and palate may occur along with other syndromes or birth defects.
- Cleft children have difficulties to ingest mother’s milk. Consequences are malnutrion and even death. Often the mother and child are neglected, even by their family members.
Some of the major barriers the families and children face with Cleft Lip or Palate are:
- Access to quality healthcare
- Unavailability of treatment at public health institutions
- Absence of services within their reach
- Poor treatment seeking behaviour
ABMSS is helping to address these issues amongst rural population sensitively and provides customized care to the most important health needs of these less fortunate children born with cleft lip and palate conditions

World Health Organisation (WHO) has recognised that non-communicable diseases, including birth defects cause significant infant mortality and childhood morbidity and have included cleft lip and palate in their Global Burden of Disease (GBD).
Ideal situation should be: Cleft treatment for the affected should start as early as possible.Some children may be lucky and need a single surgery; otherwise these interventions might take more than one surgery as mentioned below:

After ruling out other anomaly, counselling and educating the parents with emphasis on feeding therapy provided. Decision made for NAM and put on periodic recall. New born hearing assessment, preventive dental advise along with speech therapy education to the mother imparted
Period: 0 – 3 months
Cheilorhinoplasty: Lip and nose repair, Speech therapy initiated with stimulation and tongue posturing till it’s time for palate closure.
Period: 3 Months +
Palatoplasty: Palatoplasty and Grommet insertion (ventilation tube) to prevent middle ear problems.
Annual ENT examination and hearing test up to 5 years of age
Period: 10 – 14 months
Speech and language therapy: Speech problems Paediatric Dentistry advice and/or treatment if necessary.
Assessment for speech problems at this age or later stages to involve Naso endoscopy/video fluoroscopy.
Surgery to revise lip, fistula closure or speech (velopharyngeal) surgery or ENT surgeries if necessary
The cleft child will receive the full assessment by the cleft team at age 5 to ensure that on-going or emerging issues are appropriately managed
Period: 18 months – 5 years
Orthodontic assessment / treatment: Arch expansion preparation for bone graft and protraction of maxilla
Speech and hearing checked and managed accordingly
Psychological support for any issues such as bullying or self-confidence.
Full assessment by cleft team at age 10
Alveolar Bone Graft (ABG) Assessment and Surgery done between the ages of 8 - 11 based on orthodontic preparation and canine root formation.
Dental treatment/ revision surgery to improve previous results is available through till the child becomes an adult along the entire pathway.
Period: 6 – 12 years
Orthognathic Surgery: Between 12-16 years orthodontic treatment is mainly rendered preparing the child for Orthognathic surgery post growth completion.
Skeletal repositioning of the jaws done mainly for the maxilla to reverse the lack of growth due to the effect of earlier surgery
Period: 16 to 18 years
Rhinoplasty: Last of the surgeries done along with any touch up required for lip or palate or the speech related procedure.
Dental treatment/ revision surgery to improve previous results is available through till the child becomes an adult along the entire pathway
Period: 18 years
But the reality is: Due to lack of awareness, unavailability of treatment at public health institutions, not getting affordable medical assistance within their reach many parents get frustrated with situation and feel depressed and isolated from immediate family, friend and community around them.
Problem in India: every year around 40,000 children are born with deformities ie, each day around 109 children are born with facial deformities. But each case is not identified and treated. As a result over ten lakh such children and adults are left unattended / untreated.
Cleft lip and/or palate is not just a deformity, it causes difficulty in breast feeding for the new born babies and later as they grow they face problem with eating, breathing, hampered speech. If left untreated the child may not survive even for a year.
For those who survive in spite of the difficulties, bad facial outlook leads to social ostracization; which leads to lack of education, isolation, and deteriorated health.
Whilst there are several organisations that fund for correcting birth defects like cleft lip and palate, there are almost none that address these problems.
Solution:ABMSS scouts the rural areas and identifies children, born with facial deformities. Once identified these children are provided complete care till they are fit to return to mainstream society.
